My GP ceased to practice at the end of 2015 with no warning to his patients in advance. I have no idea if he retired or was unwell or even deceased. I would only visit him about twice a year and had no idea that he was no longer practicing until I tried to call to make an appointment and the line was dead. The new shop on the same premises has no forwarding information for him. How can I obtain a copy of my medical records?
