Should read "what happens" - basically the GP who provides care for my two elderly relatives, his partner GP, his nurse and all the staff at the surgery provide an excellent service everyday.
They are unsung heroes and may often slip through the cracks on the acknowledgement of service ladder due to the "day-to-day" care they provide and not the "major crisis" response that may bring other services to the notice of the general public or media.
Their consorted efforts, time, patience, pleasant manner and of course clinical excellence I believe avert (certainly for my family) patients reaching crisis point.
